Lights of Remembrance

Ohio’s Hospice is honored and privileged to offer the communities it serves a memorial tree
during the holiday season with our Pathways of Hope Lights of Remembrance.
The memorial trees are an opportunity for those in the community
to remember a loved one who has died.

Please complete the form below and submit to have an angel ornament placed
on the memorial tree in memory of your loved one.

Trees will be on display through Dec. 31, 2024.
