In consideration of the acceptance of my entry, I hereby waive, discharge and release on behalf of my heirs, executors, and assigns, all claims of any nature, including but not limited to damages, actions, whatsoever in any manner arising from my participation at the Ohio's Hospice of Miami County 3K/5K Memorial Walk, and do hereby release the City of Troy, Ohio's Hospice of Miami County and employees, trustees, sponsors, workers, officials, agents, independent contractors and volunteers from any claim whatsoever arising from this event. I agree to abide by all the rules of participation and acknowledge that Ohio's Hospice of Miami County 3K/5K Memorial Walk has the right to refuse or return my entry at its discretion. I attest and verify that I understand the risks involved in such a run/walk and that I am physically fit and have trained adequately in preparation. I give Ohio's Hospice and its assigns permission from or acting for the benefit of Ohio's Hospice to take, show and reproduce my name, photograph and picture during the event and use it in its publicity, media, and marketing material at any time and for any reason. I have read and agree to the terms of the waiver on this form and certify compliance by the signature.
